We present an elusive case of ERCP for choledocholithiasis where was unsuccessful due the patient’s anatomic orientation. The patient 61-year-old Caucasian male who admitted epigastric, right upper quadrant pain, nausea. Abdominal imaging revealed at least 2 stones in distal common bile duct. Upon evaluation his management plan included as had periampullary diverticulum making it difficult visualize ampulla. Ultimately, next step follow up laparoscopic cholecystectomy (LC) alleviate pain symptoms. Postoperatively stable doing well on up. referred tertiary care center further postoperative ERCP. Although, interventions attempted this allowed him become stable, need advancements must be established patients anatomical challenges order prevent repeated procedures relating delayed removal gallstones.
